Health Form Instructions
You will receive a letter with detailed instructions in a later mailing.
In early July, you will have access to your Magnus Health SMR account through OnCampus (Hamden Hall Country Day School’s Learning Management System). You will access the account via the “Magnus Health Portal” icon located within the Resources tab. We ask that you enter the health information required within your Magnus Health SMR account for each child attending Hamden Hall no later than mid-August.
Annual Physical Form- all new students entering Hamden Hall (HH) for 2025-2026 need to submit the State of CT Department of Education Health Assessment Record (available to download from the Magnus Health Portal). If you have any questions, please contact our school nurse, Claudia Murtagh, at cmurtagh@hamdenhall.org.
